Position Summary

Works in collaboration with a physician and other Alivia Supportive Care team members in an active practice to deliver palliative care and related health care services to patients and families in patient homes and a variety of other settings.

Westside and Clay County

Shift: Mon-Fri

Essential Functions

  • Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation, and education for both patients and family
  • Assists with diagnosis, treatment, and management of acute and chronic health conditions
  • Orders, performs, and interprets laboratory and radiology tests within scope of professional practice
  • Writes orders pertaining to informed consents, diagnostic procedures, and treatments
  • Prescribes medications and other treatments
  • Consults with palliative care physician or designee as needed, informs attending physician of services provided and collaborates with other physicians as needed.
  • Facilitates clarification of patient and family goals of care.
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement of services provided
  • Executes physician’s orders in accordance with legal, ethical, and organizational standards
  • Complies with applicable laws and regulations
  • Initiates reimbursement for services rendered on a timely basis
  • Ensures provision of informed consent for CPR and DNR
  • Recommends medications as required to treat specific populations
  • Adheres to the practice of confidentiality regarding patients, families, staff, and the organization
  • May provide educational sessions to advance palliative medicine among internal staff, medical community, and community as a whole
  • Participates in on-call coverage
  • Participates in regular interdisciplinary team meetings to review active cases
  • Performs other duties as needed

Qualifications

Knowledge and Critical Skills

  • Patient care charts and patient histories
  • Health education theory and practice
  • Clinical operations and procedures
  • Current principles, methods, and procedures for the delivery of medical evaluation, diagnosis, and treatment in area of expertise
  • Excellent interpersonal and communications skills

Experience

  • Minimum of two-years nursing experience
  • Experience in hospice palliative care, preferred

License / Certification

  • Advanced Practice Registered Nurse (APRN), Nurse Practitioner (NP), or Certified Registered Nurse Practitioner (CRNP) Certification
  • Valid State of Florida Driver’s license with driving record acceptable to Community Hospice insurance carrier.
  • Advanced Practice Nurse or PA licensed in the State of Florida

Education

Other Requirements

  • Medical clearance for communicable diseases and current immunizations required before participant contact.
