Age Well is seeking a Nurse Practitioner to join our growing practice in Jacksonville, FL. The selected Advanced Practitioner Provider would serve as a link between inpatient and outpatient care as a member of the Baptist Health team.

Job Responsibilities- Consults with patients about their current conditions, orders diagnostics, analyzes results, diagnoses conditions, and prescribes treatment strategies, including prescription medications.
- May also actively participate in research in the field and advocate for policies that support the needs of elderly patients.
- Master's Degree (MS) in Nursing
- Licensed Advanced Practice Registered Nurse (APRN)
- APRN Experience
- Driver's License - DMV
- Advanced Registered Nurse Practitioner - State Nursing Boards
- DEA Controlled Substances Registration - DEA
