Signify Health is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, visiting patients in their own homes to bridge gaps in care. Each visit lasts about 45 minutes and includes health history review, exam, and optional tests as ordered by the plan.
Active, unrestricted licenses and board certification are required, with multi-state licenses preferred. Travel to local licensed states and occasional virtual visits are expected.
