Csnf is currently recruiting a Nurse Case Manager for our busy Specialty Practice offering a generous sign-on bonus. The Nurse Case Manager role integrates case management, patient education, symptom management, and telephonic triage to ensure timely, safe, and effective care delivery. Collaborates with physicians, mid-level providers and all members of the multidisciplinary team within Csnf and local hospitals to ensure that patients and their families receive services which reflect our commitment to caring for the body, mind and spirit.
Essential duties and responsibilities include the following:
- Coordinate same-day infusion visits for urgent supportive care and provide clear patient instructions/reinforce regarding when to call back or escalate care.
- Monitor canopy work queue ticket dashboard. Prioritize patient needs and determine appropriate level of care, provide real-time clinical advice for symptom management, and escalate urgent or complex cases to providers promptly.
- Utilize canopy symptom algorithms to perform telephone triage of patient-reported symptoms, document symptom related calls, and coordinate care with providers and clinic staff. Enter verbal orders as per provider or utilize standing orders.
- Act as liaison between patients/caregivers and multidisciplinary teams when coordinating care. This includes, but is not limited to, providers, financial counselors, schedulers, medical assistants, clinic nurses, authorization staff, dieticians, social workers, financial counselors, and pharmacists.
- Complete detailed documentation of all patient interactions and education related to care and communications with patients, staff and providers.
- Follow up with patients when needed to ensure resolution of documented issues.
- Monitor hie (health information exchange) notifications and follow up with patients/caregivers to coordinate follow up visits, ensure patient needs are met, and download any appropriate hospital medical records into clinic emr. Monitor hospitalization and re-admissions as related to case management.
- Maintain oncology related competency via required ons courses and ceus. Demonstrate competency to provide care appropriate to the age group of this patients' population.
- Maintain hipaa compliance and patient confidentiality.
- All other duties as assigned

Education and experience:
- Minimum of 2 years oncology experience required
Certificates, licenses, registrations:
- Current, unincumbered rn license in the state of florida
- ocn preferred
- bsn preferred
