Job Summary:Lutheran Haven is seeking a Licensed Practical Nurse to join the care team. The LPN is expected to assist in maintaining a positive physical, social, and psychological environment in accordance with the residents’ needs and facility philosophy. The LPN is responsible to the Resident Care Coordinator for the planning, directing, supervision, and delivery of direct resident care. Additionally, the LPN is responsible for maintaining accurate and complete medical records, utilizing safety measures in provision of care, and prevent spread of disease and infection by following established universal precautions.

  • Actively supports Lutheran Haven’s Mission and Ministry.
  • Demonstrates a willingness to be an effective team member by fostering positive relationships with teams and residents.
  • Provides consistent leadership to mentor and motivate staff towards total quality resident care.
  • Visits with each individual resident three (3) times per week (generally Tuesday, Wednesday, and Thursday) to address any needs they may have and maintain communication with Resident Care Coordinator on resident requests to be addressed by Resident Care Coordinator.
  • Supervises daily functions of the Assisted Living Facility (ALF), from employee supervision to delivery of resident care.
  • Gives/receives report upon reporting in and ending shift duty hours.
  • Makes initial rounds at beginning of shift on all assigned residents. Maintains awareness of comfort and safety needs of residents.
  • Communicates immediately to supervisor/administrator on call any challenges requiring immediate attention.
  • Administers personal hygiene, preventive, therapeutic and rehabilitative services to residents and assists in the coordination of medical care with care rendered by the other disciplines.
  • Admits new residents by evaluating/assessing their needs and reviewing physician orders.
  • Orders prescribed medications, supplies and equipment as necessary, in accordance with established policies; also assists residents to self-administer prescribed medications, performs procedures and treatments in accordance with limited nursing services approved nursing techniques. Maintains security of all medications including narcotics in accordance with facility policy and regulations.
  • Requisition s and arranges for diagnostic and therapeutic services as ordered by the resident’s physician.
  • Maintains documentation which accurately and completely records data pertaining to resident’s care, condition, and treatment in accordance with HIPAA.
  • Enters accurate information from resident’s physician order sheet and medication record into the “medical record” component on the pharmacy Medication Observation Record at the time of resident discharge or transfer.
  • Other duties as assigned.

Minimum Qualifications:

  • Must be currently licensed by the State of Florida as a Licensed Practical Nurse.
  • Must have a minimum of one year of verifiable, successful clinical experience; experience in geriatrics or rehabilitation environment preferred.
  • Must have a current CPR card.
