Hospice of Florida is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our nursing team delivering compassionate end-of-life care across Jacksonville. The role features a Monday–Friday, 8 am–4:30 pm schedule and a territory focused on Jacksonville.
As an RNCM, you will provide hands-on nursing care, coordinate services, and guide patients and families through the hospice journey, working with an interdisciplinary team to ensure comfort, dignity and high-quality care.
