We’re Hiring: PRN Family Nurse Practitioner

Caring Heart Family Nurse Practitioner (CHFNP) is seeking a licensedFlorida-licensed Autonomous Family Nurse Practitioner (FNP) to join our team on a per diem/PRN basis, providing primary and preventive care to patients in Palm Coast and Daytona, Florida.

This role supports CHFNP’s mission of delivering accessible, high-quality care to underserved, homebound, elderly, and assisted living facility (ALF) patients. Assignments and shifts are scheduled flexibly based on patient volume and coverage needs within each service area.

Roles & Responsibilities

Clinical Care

• Conduct comprehensive history and physical examinations for patients in the home, ALF, and/or office setting, as assigned.
• Evaluate, diagnose, and manage acute and chronic conditions within the FNP’s scope of practice.
• Develop and implement individualized treatment and care plans, including medication management.
• Order, perform, and interpret diagnostic tests, including laboratory tests, EKGs, Holter monitoring, and point-of-care testing, as clinically indicated.
• Coordinate care with collaborating specialists, including cardiology, urology, and other referral partners, as needed.
• Provide compassionate care with empathy, kindness, and respect.
• Complete all clinical documentation and charts within 24–48 hours of the patient encounter.

Requirements

• Active Florida APRN license.
• Active Florida Autonomous APRN registration/authorization and eligibility to practice autonomously in the applicable primary-care setting.
• Family Nurse Practitioner (FNP) certification.
• Must be authorized to practice within the applicable Florida APRN scope and meet all requirements for the position.
• Professional liability/malpractice insurance as required for the role.
• Ability and willingness to provide care in patients’ homes and ALF settings.
• Strong clinical assessment, communication, and documentation skills.
• Reliable transportation and willingness to travel within the assigned service areas.
• Background check required as part of the hiring process.
