LTSS Service Coordinator (Bilingual in Spanish/English)
Schedule: Monday-Friday 8am-5pm EST
Location: Primarily a field-based position. Candidates must reside in one of the following counties (Region
B):

Field: The role involves predominantly field operations, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training.
Responsibilities:
- Conduct face‑to‑face program assessments using pre‑defined tools and motivational interviewing techniques for evaluation, coordination, and management of an individual’s waiver (such as LTSS/IDD) and related health needs.
- Identify members with potential clinical health care needs and coordinate those cases with clinical health management and interdisciplinary teams.
- Manage non‑clinical needs of members with chronic illnesses, comorbidities, or disabilities, ensuring cost‑effective and efficient utilization of long‑term services and supports.
- Document short and long‑term service and support goals in collaboration with the member’s chosen care team.
- Identify members who may benefit from an alternative level of service or other waiver programs.
- Serve as a mentor or preceptor for new staff, assist in formal training, and support process improvement initiatives.
- Submit utilization/authorization requests to utilization management with supporting documentation aligned with the individual’s care plan.
- Report critical incidents to appropriate internal and external parties such as state and county agencies.
- Assist and participate in appeals, grievances, and state audits.
Minimum Requirements:
- BA/BS degree and at least 2 years of experience working with a social work agency, or equivalent education and experience.
- Education, years, and type of experience may vary based on state law and contract requirements.
Preferred Skills, Capabilities, and Experiences:
- BA/BS degree in a health‑care related field.
- Bilingual in English/Spanish.
- Experience in LTSS, case management, social work, or hospital discharge planning.
