Carteret Management Corporation is seeking a bilingual RESIDENT SERVICE COORDINATOR to assist senior individuals and persons with disabilities living in Cathedral Tower , an affordable housing community located in downtown Jacksonville, to obtain needed supportive services from community agencies. This position offers full-time employment at a competitive salary with a strong benefit package including:

Responsibilities:

* Provide general case management (including intake) and referral services to all residents needing assistance.

* Establish linkages with all agencies and service providers in the community.

* Set up a directory of providers for use by both project staff and residents.

* Refer and link the residents to service providers in the general community.

* Educate residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.

* Develop case plans in coordination with assessment services in the community or with a PAC.

* Monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual.

* Set up volunteer support programs with service organizations in the community.

* Help the residents build informal support networks with other residents, family and friends.

* Educate other staff on the management team on issues related to aging in place and service coordination to help them to better work with and assist the residents.

* Maintain detailed resident files.

Requirements

* Excellent customer service skills.

* Experience with seniors a plus.

* A Bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable; individuals without a degree but with appropriate work experience may be hired.

* Two to three years experience in social service delivery with senior citizens and non-elderly disabled in the area served by the project.

* Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled in the area served by the project.

* Knowledge of referral processes.

* Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled served.

* Proficiency with computers, including Microsoft Word, Excel and Outlook.

* Ability to work cooperatively and effectively with various groups (on-site staff, vendors, residents).
