Position Summary
The Care Coordinator is responsible for supporting patients and families by coordinating healthcare services, ensuring smooth transitions of care, and promoting adherence to treatment plans. This role involves working closely with providers, payers, and community resources to deliver patient-centered care.
Essential FunctionsCare Management
- Monitor patient progress and address barriers to care.
- Facilitate communication between patients, clinical teams, and external partners.
Documentation & Compliance
- Maintain accurate records in care management systems and EHR.
- Ensure compliance with organizational policies and regulatory requirements.
- Assist patients’ parents in understanding their care plans and available resources.
- Coordinate appointments, referrals, and follow-up care across multiple providers.
Resource Coordination
- Connect patients with community services, financial assistance, and support programs.
- Educate patients on preventive care and wellness initiatives.
- Strong communication and interpersonal skills.
- Ability to multitask and manage time effectively.
- Proficiency in care management software and electronic health records (EHR).
- Core competencies:
empathy and patient advocacy, organizational skills, collaboration and teamwork, and problem solving.
- Bachelor’s degree in nursing, Licensed Practical Nurse certification, or a related field is preferred.
- Minimum of 1-2 years of experience in care coordination, case management, or patient services preferred.
Direct supervision of Care Coordination staff.
Working ConditionsGeneral office working conditions. May require occasional overtime during peak periods, such as month-end or year-end closings.

Frequent travel is required, often up to several hours of driving per day. Ability to travel to, attend, and conduct sales presentations. Manual dexterity is required to use desktop computers and peripherals. Exposure to variable weather conditions is likely.
