Social Work Services Program Consultant for the Medical Foster Care program in the Florida Department of Health, based in Jacksonville. The role involves managing referrals, conducting assessments, ensuring compliance with program standards, recruiting and training medical foster parents, and coordinating care across the medical foster care system in a five-county service area. The position reports to a nursing supervisor and requires regular home visits and travel.
Keyqualifications:
- Valid driver's license and willingness to complete home visits with travel up to 30% of scheduled hours
- Ability to work with families and children with special health care needs
- Strong organizational, time management, and communication skills (verbal and written)
- Ability to use computer systems including Microsoft Office, electronic health records, and virtual meeting platforms
- Preferred:
Bachelor's degree or higher in social services or related field; 1-3 years of child welfare and medical setting experience
