We are currently seeking a compassionate and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Wound Care Nurse. • Assess complex patient wounds upon admission or development
• Clean and dress wounds to promote healing and prevent infection
• Determine appropriate treatment plans, which may include antibiotics, hyperbaric therapy, or surgery
• Educate patients and caregivers on wound care post-discharge
• Provide verbal instructions and document care plans clearly
• Complete all required documentation for patient records and insurance purposes
• Collaborate with a team of caring professionals to provide high-quality, consistent care

• Licensed LPN or RN in the state of Illinois
• Reliable, compassionate, and team-oriented
• Committed to delivering exceptional care in a skilled nursing environment
• Enthusiastic about working in a supportive, community-centered facility
