Pyramid Healthcare is seeking a dedicated Registered Nurse (RN) to join our Assertive Community Treatment Team (ACTT) in Jacksonville, NC. This community-based role is ideal for an RN who is passionate about serving adults living with severe and persistent mental illness while helping reduce hospitalizations and improve long-term recovery outcomes.
Our ACTT program is often described as a "hospital without walls," delivering intensive behavioral health services directly in the community. As part of a multidisciplinary team, you'll work alongside psychiatrists, therapists, Qualified Professionals, peer support specialists, and case managers to provide individualized, person-centered care where clients live, work, and thrive.
What You'll Do- Coordinate medical care with physicians, hospitals, pharmacies, and other healthcare providers
- Monitor medication compliance and administer injectable medications (IM injections) as prescribed
- Maintain medication inventories and coordinate prescription refills
- Assist with physician appointments, obtain vital signs, and maintain accurate clinical documentation
- Complete AIMS assessments upon admission and every six months, as required
- Educate clients and staff on medications, treatment plans, and overall wellness
- Provide transportation for clients to medical appointments and other community resources as needed
- Deliver nursing services in community settings, including clients' homes and other locations throughout the service area
- Participate in an on-call rotation that may include crisis intervention, after-hours assessments, hospital diversion, and commitment procedures
- Collaborate with the ACTT team to develop and implement individualized treatment plans
- Complete additional duties that support quality client care and recovery
- Current, unrestricted Registered Nurse (RN) license in North Carolina
- Strong assessment, documentation, and care coordination skills
- Experience in behavioral health, psychiatric nursing, community mental health, or substance use treatment preferred
- Valid driver's license with an acceptable driving record
- Ability to travel throughout the community to provide client services
