Responsibilities- Conduct adult and geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Coordinate with the primary care physician on patient education and follow-up as necessary.
- Provide services at the member’s place of residence, which may be a home or a skilled nursing facility.
- Perform the Matrix Comprehensive Health Assessment (CHA) to identify care gaps and improve overall health outcomes.
- Flexibility – Visits are scheduled based on your availability. We conduct assessments Sunday through Saturday from 8 am to 7 pm. You can work during the day, evening, or weekend.
- Competitive Pay – Earn income based on the number of assessments completed. Pay ranges from $90 to $115 per assessment, with potential earnings of $1,800 to $11,500 per month for 20 to 100 visits.

- Education and Support – Comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education.
- Additional Licensure – Support for obtaining additional state licensure and credentialing in neighboring or desired states.
- Full-time Opportunities – PRN providers can transition to full-time roles, receive a sign-on bonus, and work in their home location or region.
- Master’s degree or equivalent experience, and satisfactory completion of NP licensure.
- Current NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority.
- Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- 1 year of NP experience preferred; new graduates encouraged.
- Valid state driver’s license for the state of residence.
- Automobile insurance coverage required for the state of residence (or ability to obtain).
- Strong computing skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
Risk coverage:
Bodily Injury: $100,000 per person/$300,000 per incident;
Property Damage: $50,000;
Collision and comprehensive deductibles: $1,000 or less.
