Associate Vice President, Community Wellness Pathways

The Associate Vice President of Community Wellness Pathways (AVPCWP) is a core member of the senior leadership team reporting directly to the CEO. This role is responsible for developing and executing the organization's strategic plan to maximize JPNDC's approach to deliver proactive and tailored social impact services to assist families and individuals in need of pathways to greater stability. It is a new position created to leverage JPNDC's recent growth to improve client and resident impact across Greater Boston and the MA landscape.

As a dedicated and skilled senior professional, the AVPCWP will serve as the agency's lead supervisor for a team of roughly six Case Managers who will serve two distinct but equally important caseloads supporting JPNDC's mission. The first focus area is our Family Childcare Program (FCC), supporting priority populations including DCF-involved families, young parents under 24, and families experiencing homelessness. The second focus area is Economic Prosperity Services (EPS), delivering support to residents within JPNDC's housing portfolio and throughout the City of Boston and beyond.

Case Managers will support the delivery of case services, housing stability plans, individualized family needs assessments, facilitate tailored Family Plans, and conduct developmental evaluations.

JPNDC has a strong coalition focus which aligns with its long legacy of organizing key stakeholders. Working closely with the Program Director of Family Child Care and the Associate Vice President of Economic Prosperity Services, the AVPCWP will lead and expand on strategic partnerships within the housing, medical, health, and human services industries to partner in coalition around approaches to better define synergies between housing and proactive service delivery.

All CWP staff members are considered mandated reporters and will be supported with the requisite training as needed. This role is ideal for an experienced social service professional who is passionate about making a positive impact in the community by supporting staff and our residents, clients, and families and children in the FCC program, particularly those with high needs facing challenging circumstances.

Duties:

  • Primary lead on high-visibility and contract cases with JPNDC residents, clients and FCC priority populations such as: DCF, Young Parents Under 24, and Families Experiencing Homelessness.
  • Serves as a resource to senior management while supporting recruitment, interviewing, hiring, and onboarding; establishes departmental standards of practice; maintains documentation; conducts psychosocial assessments and develops care-plan goals; and ensures resident and family participation in the care-planning process.
  • Oversee HIPPA regulated client files
  • Regularly coordinate with the member of senior management
  • Represent the JPNDC in regular meetings with Housing Court, DCF, Shelter Systems or other family service agencies.
  • Support the contractual needs of the children and families including referrals to community resources and multi-agency reporting as required.
  • Develop strategies for cross department referrals with JPNDC Economic Prosperity Services to develop Individualized Family Plans and goal setting plans tailored to each family's needs and goals.
  • Oversee staff and perform developmental evaluations such as: ASQ's/ASQ-SE's for newly enrolled and families as needed.
  • Develops curricula and co-facilitates parent workshops, education, self-sufficiency, and other related enrichment activities for priority population.
  • Provide families with high level and ongoing support in navigating referral processes such as:
    Early Intervention, Public School Systems, Public Assistance Benefits, and/or Hospitals.
  • Provide one-on-one assistance and referrals to senior staff to help identify high-needs families.
  • Conduct routine home visits to educator programs and prepare related reports.
  • Other duties as assigned by supervisor.

Qualifications:

Required:

  • Current and Active LICSW.
  • Master's degree in Social Work or a related Human Services Degree
