Responsible for coordinating, planning and documenting patient care staff and patient schedules. Performs functions, in order to improve the clinical, operational and financial success of the agency.

QUALIFICATIONS
  • An LPN currently licensed by the state in which employed by the agency.
  • At least one year of experience in home health care preferred.
  • Requires excellent interpersonal skills and communication, as well as the ability to effectively interact with all levels of management.
  • Must have strong organizational skills and be able to effectively manage and prioritize tasks and be time efficient.
  • Computer experience necessary to perform job responsibilities.
  • Good working knowledge of the patient Referral process.
KEY RESPONSIBILITIES
  • Receives/responds to incoming telephone calls from referral sources/patients. Obtain patient orders, in order to process the Referral.
  • Establish patient and employee schedules by coordination of patient’s needs and staff availability. Process schedules routine visits, HUVs & SFVs. Re-schedules, re-assignments and declined visits.
  • Input all essential patient and employee schedule information in EMR in an accurate and timely manner. Complete all requests according to priority and necessity of the patient’s needs.
  • Coordinate schedule of patient and relays it to the appropriate party, so that optimal patient care will be provided.
  • Maintain a current and accurate record of direct clinical staff availability and coordinates assignment of nursing personnel according to specific patient needs and requests.
  • Communicate all problems and concerns of patient services and staff employee performance/issues to the appropriate person.
  • Reschedule staff employee schedules according to illnesses, emergencies, etc. Inform all clinical staff of any change in scheduling.
  • Produce reports as requested by appropriate personnel
  • May attend PI Utilization Review Committee on a quarterly basis or as scheduled.
  • Organizes, prioritizes and executes work assignments effectively and efficiently.
  • Receives instructions and keeps ongoing contact with appropriate personnel.
  • Attends case conferences, in services, seminars as scheduled, and other meetings as assigned.
  • Maintains confidentiality of information related to business practices, business activities, and personnel.
  • Maintains a professional attitude when working with staff, fostering the team philosophy and team environment for all personnel.
  • Does not allow personal affairs to interfere with scheduled work time.
  • Maintains a professional appearance at all times.
  • Provides proper notification and/or advanced notice of absence or tardiness without abuse.
  • Participates willingly with special projects and/or overtime work when requested.
  • Follows all agency policy and procedures.
