Overview
Great Lakes Physician Practice (GLPP) Family Practice is hiring a Full‑Time Nurse Practitioner to join their outpatient team. The practice operates two office locations, Jamestown and Salamanca, with a schedule of 8am–5pm.
Responsibilities- Apply professional practice knowledge to provide patient care activities for a group of patients and their families, using independent judgment and collaborating with all team members.
- Develop, implement, and evaluate individualized plans of care that consider each patient’s needs and the organization’s quality, safety, and satisfaction goals.
- Communicate effectively with patients, families, and health‑care providers through verbal interaction and accurate documentation.
- Demonstrate critical thinking across all areas of professional practice and seek guidance to continuously improve clinical practice.
- Establish and maintain collaborative relationships with physicians, other health‑care providers, patients, and families to achieve desired patient outcomes.
- Engage in the clinical development of all team members, requesting opportunities to learn advanced techniques and utilizing performance feedback to improve practice.
- Provide basic education to patients and families to support the episode of care and promote health‑promoting behaviors.
- Participate in quality improvement initiatives, identify opportunities for improvement, and make recommendations for the enhancement of clinical care.
- Successful completion of an approved nurse‑practitioner program.
- BSN required; MSN preferred.
- Current New York State RN license and NP certification.
- Current CPR certification;
Acute Cardiac Life Support (ACLS) or Basic Life Support (BLS) preferred. - Professional nursing experience and/or nurse‑practitioner experience preferred.
- Ability to work productively in a complex environment, handle multiple priorities, and adapt to changing tasks.
- Strong critical‑thinking, analytical, and problem‑solving skills.
- Excellent clinical judgement and interpersonal skills.
- Physical stamina for frequent walking, standing, lifting, and positioning of patients.
