Adams Farm Living and Rehabilitation, part of the Century Care family, is currently hiring! Our facility provides residents with a full range of long-term care and rehabilitation services, along with a variety of engaging social and recreational activities. We currently have an opportunity for new staff to be part of our team in our Greensboro facility.

PositionRN MDS Coordinator
The Care / MDS Coordinator is responsible for timely and accurate completion, coordination, recording, updating, and monitoring of all Minimum Data Set (MDS) documentation required by state and other appropriate authorities.
Responsibilities- Provide clinical and administrative support to the Director of Nursing Services
- Regulate and maintain the MDS/PPS program and completion schedule
- Develop and coordinate appropriate nursing care plans for all residents
- Evaluate effectiveness of nursing care plans, and update MDS/RAI process within completion guidelines
- Work in collaboration with the Interdisciplinary Team to assess the needs of the resident
- Monitor MDS and care plan documentation for all residents. Ensure documentation is present in the medical record to support MDS coding.
- Required - Active and unencumbered NC License as an RN
- Prior MDS and Case Mix experience is preferred.
- Case management experience is preferred
- Experience with Electronic Health Records, Point Click Care is preferred
- Ability to communicate effectively with residents and their family members, and at all levels of the organization
- Ability to organize and prioritize
- Ability to maintain confidentiality
- Ability to be accurate, concise and detail oriented
- Knowledge of RAI requirements is preferred
