Adams Farm Living and Rehabilitation, part of the Century Care family, is currently hiring! Our facility provides residents with a full range of long-term care and rehabilitation services, along with a variety of engaging social and recreational activities. We currently have an opportunity for new staff to be part of our team in our Greensboro facility.

RN MDS Coordinator

The Care / MDS Coordinator is responsible for timely and accurate completion, coordination, recording, updating, and monitoring of all Minimum Data Set (MDS) documentation required by state and other appropriate authorities.

• Provide clinical and administrative support to the Director of Nursing Services
• Regulate and maintain the MDS/PPS program and completion schedule
• Develop and coordinate appropriate nursing care plans for all residents
• Evaluate effectiveness of nursing care plans, and update MDS/RAI process within completion guidelines
• Work in collaboration with the Interdisciplinary Team to assess the needs of the resident
• Monitor MDS and care plan documentation for all residents. Ensure documentation is present in the medical record to support MDS coding.

• Required - Active and unencumbered NC License as an RN
• Prior MDS and Case Mix experience is preferred.
• Case management experience is preferred
• Experience with Electronic Health Records, Point Click Care  is preferred
• Ability to communicate effectively with residents and their family members, and at all levels of the organization
• Ability to organize and prioritize
• Ability to maintain confidentiality
• Ability to be accurate, concise and detail oriented
• Knowledge of RAI requirements is preferred
