- Salary Range: $34.00 - $51.00 per hour. $10,000 sign‑on bonus available for qualified candidates

Integrates health care for patients/clients across the continuum of care and collaborates with the health care team and other professionals to coordinate and ensure timely sequencing of care. Enhances patient flow and interdisciplinary communication to promote early intervention, and removes barriers of care for patients, clients, and family members.
Responsibilities- Integrates health care for patients/clients across the continuum of care and collaborates with healthcare team and other professionals to coordinate and ensure timely sequencing of care.
- Supports patients/clients and family members by removing barriers of care and evaluating outcomes based on integration among established clinical, financial, and utilization data.
- Serves as key link between providers, clinics, insurance concerns, communication, inpatient floors and outside facilities.
- Maintains good interpersonal relationships with partners and coordinates, facilitates and negotiates with others.
- Monitors, evaluates and trends patient responses using structured care methodologies while working closely with ancillary and support services.
- Identifies and implements process improvements when needed, is self‑motivated, and works with little direction.
- Facilitates groups, multidisciplinary teams, and implements best practices when necessary while functioning within the scopes and standards of nursing practice as outlined in the Nurse Practice Act.
- Applies age‑related patient care protocols pertaining to the physical and psychological needs of pediatric, adolescent, adult, and geriatric patients as per department scope of service.
- Bachelor’s degree in Nursing strongly preferred (Bemidji, Rapid City) and required in Fargo, Sioux Falls, Bismarck (within one year of hire with completion within three years). Waivers may be considered under certain conditions.
- Graduate from an accredited nursing program, including CCNE, ACEN, or NLN CNEA.
- Minimum of two years clinical experience preferred.
- Current unencumbered RN license with the State Board of Nursing where practice occurs; multistate licensure acceptable for Nurse Licensure Compact states.
- Must obtain and maintain required department‑specific competencies and certifications.
