Hospice Social Worker – Day Shift (8 Hours)

Contract - 13 Weeks

Shift: Monday–Friday | 8:00 AM – 4:30 PM

Hours:

40 hours per week

Contract:

Guaranteed hours per contract week

We are seeking an experienced Hospice Social Worker to support terminally ill patients and their families through the end‑of‑life process. The ideal candidate will be comfortable performing initial assessments, routine visits, and providing emotional and practical support in a hospice setting.

Patients have a terminal diagnosis with a prognosis of six months or less.

• Patient population:
  Hospice / End‑of‑Life care
• Average caseload: 35–40 patients per social worker
• Daily visits: 4–5 visits per day

• Days:
  Monday through Friday
• On‑call:
  None
• Holidays:
  None

Minimum experience:

Hospice experience preferred

First‑time travelers:
Not accepted

MSW or BSW required

• Perform initial psychosocial assessments and routine hospice visits
• Provide emotional and practical support to patients and families during the dying process
• Knowledge of community resources
• Assist families with Power of Attorney and advance care planning
• Support and participate in DNR conversations
• Strong communication and documentation skills
