Position Summary and Job Duties

Under the direction of the program supervisor, this role provides comprehensive, person‑centered, team‑based, assertive community treatment to persons living with severe and persistent mental illness who receive services through the Community Support Program. Duties include case management, symptom management, supportive psychotherapy, psychosocial rehabilitation, vocational supports, and assistance with activities of daily living according to individualized treatment plans. The candidate provides a variety of therapeutic interventions for persons with mental illness and/or co‑occurring disorders based on clinical assessments, and develops and maintains client case records, benefits, plans, and reports in accordance with DHS 63, Social Security, and Medicaid/Medicare requirements.

The work is conducted in collaboration with a treatment team, other HSD employees, families, and collateral agencies.

Responsibilities
  • Manage a caseload and provide or arrange for services for each client, developing and implementing treatment plans that are culturally competent, recovery‑oriented, and involve consumer participation.
  • Provide interventions such as symptom management, supportive psychotherapy, psychosocial rehabilitation, vocational supports, and assistance with activities of daily living on individual and group levels, including supportive services and education to families, and work with clients who have more intensive clinical needs.
  • Advocate for and arrange transition to the least restrictive living situation for all CSP consumers; attend and testify at court proceedings as needed.
  • Serve as a team member on a multidisciplinary team, contributing to team functioning and covering for other members as necessary.
  • Regularly assess consumer mental status, areas of functioning, and symptom patterns, and communicate this assessment to the team.
Success Factors (KSA's)
  • Knowledge of assessment and treatment of persons with serious and persistent mental illness, including co‑occurring disorders.
  • Knowledge of social work principles and best practices, and the ability to implement them.
  • Ability to embrace and nurture a person‑centered, trauma‑informed, recovery‑oriented framework and culture for service delivery.
  • Extensive knowledge of therapeutic and psychosocial interventions for persons with complex needs.
  • Knowledge of laws, regulations, and practices related to DHS 63.
  • Knowledge of psychotropic medications, their effects, side effects, and abuse potential.
  • Knowledge of community resources and how to effectively utilize them.
  • Ability to plan and organize work effectively.
  • Ability to maintain accurate current records and prepare clear and concise reports.
  • Ability to establish and maintain effective working relationships with agency staff, community members, families, and collateral agencies.
  • Knowledge of various cultures and diverse populations and how cultural values impact individuals, families, and the community.
  • Ability to make assessments and provide or refer for appropriate treatment.
Job Requirements , Education, Training & Experience
  • Master’s Degree in Social Work or a related field from an accredited institution.
  • 1,500 hours in a CSP or 3,000 hours working with persons with severe and persistent mental illness.
  • Valid driver’s license and auto insurance; must maintain a current unrestricted license and meet Rock County policy requirements.

Physical Elements
  • Ability to stand, walk, and sit; physical movement occurs majority of the day.
  • Ability to enter client homes without assistance, including navigating stairs.
  • Work in indoor and outdoor environments with exposure to weather conditions.
  • Potential contact with unpredictable and aggressive individuals.
  • Willingness to transport consumers in a personal vehicle and operate in challenging environments.
  • Participation in disaster evacuations and fire drills as required; maintain safety procedures.
  • Lifting and carrying up to 25 lbs on occasion.
Equipment Use
  • Ability to use a computer and keyboard.
  • Become proficient in a computerized documentation system within 2 months of hire, including Microsoft Word.
  • Use of a copier, fax machine, and telephone.
Working Conditions
  • Work in multiple locations, including office, client homes, and community sites.
  • Available to work a flexible schedule, including evenings, to meet client needs.
  • Flex time when needed to coordinate emergencies, care, and recovery plan services.
  • Potential exposure to physically or verbally abusive individuals.
  • Potential exposure to unsanitary homes and high‑stress environments.
