ESSENTIAL DUTIES AND RESPONSIBILITIES
- Operates in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying the patient's behavioral/mental health problem.
- Assists in assessing participants for continuing treatment needs, prepares patient histories, discharges, and creates treatment plan of care.
- Assists in assessing patient needs and develops appropriate dispositional plans and coordinates care with the appropriate departments.
- Maintains strong communication and relationship with staff (interdepartmental and within program) and Psychiatrists.
- Secondarily, works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.
Bachelor's Degree in mental health or behavioral health field. Working toward a higher degree and/or licensure is preferred.
CERTIFICATION AND LICENSURETACT Level 2 certified within 90 days after hire. CPR/BLS preferred.

To perform this job successfully, an individual should have knowledge of database software, internet software, word‑processing software and Microsoft Outlook.
SPECIAL PHYSICAL DEMANDSWhile performing the duties of this job, the employee is regularly required to talk or listen. The employee is frequently required to stand; walk; use hands, fingers and arms to feel and reach. The employee is frequently required to sit; stoop; kneel; crouch; crawl; taste and smell. The employee must regularly lift and or move up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision and depth perception.
Specific fine motor abilities required include writing, keyboard entry and mouse use.
