Position: Children's Long-Term Support - Paraprofessional

Children's Long-term Support (CLTS) Program Assistant promotes the Mission, Vision and Guiding Principles of the Human Services Department (HSD). They perform in manner consistent with the HSD Professional Standards and Commitment to My Co-Workers. The CLTS waivers give the county and state the flexibility to develop and implement creative alternatives to placing Medicaid-eligible individuals in hospitals, nursing facilities or intermediate care facilities for persons in three target groups;

physical disabilities, developmental disabilities, and/or severe emotional disabilities. The Home and Community Based (HCBS) waiver program recognizes that many individuals at risk of being placed in these facilities can be cared for in their homes and communities, preserving their independence and keeping ties with family and friends. This position is responsible for providing customer service support to families, community members, HSD and CLTS staff members.

The major goals of this position are to answer phones, emails and process referrals, provide assistance with enrollment requirements, and provide support for CLTS program requirements.

40% – Provide information, assistance, outreach to families that contact the program for information and answer referral phone calls.
  • Answer phones and emails daily and return phone calls timely, providing information, assistance, and outreach to families that contact the program for information.
  • Gather information to start the referral process and prepare the referral for assignment; provide customer service and resources to families.
20% – Complete required documentation
  • Assist with functional and financial eligibility for the programs.
  • Assist with monthly collateral contacts, six‑month reviews, and recertification.
  • Document all client‑related activity in Avatar.
  • Complete third‑party administrator (TPA) authorizations.
  • Assist with fingerprinting and set up individual providers with the fiscal intermediary on behalf of the client employer.
20% – Collaborates with internal and external partners to respond to questions and outreaches
  • Gathering, sharing, and developing resources.
  • Community events and informational meetings.
  • Team meetings.
20% – Special projects and other duties as assigned

Qualifications
  • Understanding of cultural competence and working with diverse populations.
  • Ability to communicate effectively both orally and in writing.
  • Ability to work in a fast‑paced, changing environment.
  • Ability to prioritize work duties and successfully meet deadlines; ability to work and make decisions independently.
  • Knowledge of county‑wide resources for children, youth, and families at risk.
  • Knowledge and ability to document and manage data in Microsoft Word, Excel, and Avatar.
  • Associate’s Degree in Human Services related field.
  • Experience with children, youth, and families involved in child welfare, juvenile justice, mental health, physical disabilities, and/or developmental disabilities systems.
  • Equipment use:
    • Ability to use a computer, computer keyboard, copy machine, and fax machine.
    • Ability to type notes daily and use computer programs such as Microsoft Word, Excel, and Avatar.
  • Working conditions:
    • Ability to work in multiple locations including the office, client’s home, and a variety of community locations.
    • Valid driver's license and insurance.
    • Available to work a flexible schedule including work into the evenings when needed to meet client needs.
    • Potential exposure to physically or verbally abusive clients.
  • Physical Elements:
    • Ability to walk, stand, sit, and be active with children and youth.
