Licensed Practical Nurse; LPN

Transitions At Home is currently seeking a compassionate and dependable full‑time Licensed Practical/Vocational Nurse (LPN) to join our home health team in the Janesville area.

• Provide skilled nursing care, including routine visits, medication management, treatments, and coordination of services.
• Counsel patients and their families in the home setting in accordance with physician orders and the plan of care.
• Work under the direction and supervision of the RN Clinical Manager.

• 40 hours per week, 8:00 am – 4:30 pm
• Monday through Friday
• Limited on‑call rotation

• $29‑32 per hour, depending on experience

• Two (2) years of nursing experience, with one year in home health preferred
• CPR certification
