Position Details
Position Title: Senior Long-Term Service Care Manager (RN)
Location: Remote Texas - Member-facing
Duration: 6-Month Contract (possibility of conversion)
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Position SummaryWe are seeking an experienced RN to serve as a Senior Long-Term Service Care Manager. In this role, you will oversee the assessment, coordination, and delivery of care services for complex, high-acuity members with medical and physical health needs. You will develop personalized care plans, ensure members receive appropriate services, and act as a key advocate and resource for members and their families/caregivers.
Key Responsibilities- Develop, implement, and update long-term care plans for high-risk members.
- Coordinate care between members, families, caregivers, providers, and community resources.
- Monitor member status, identify changes in condition, and adjust care plans as needed.
- Facilitate education for members and caregivers regarding conditions, benefits, and available resources.
- Conduct home or site visits (monthly or more, as required).
- Ensure compliance with state, federal, and clinical guidelines through accurate documentation.
- Partner with leadership to improve service quality and outcomes.
- May serve as a preceptor/mentor for new clinical hires, supporting onboarding and training.
- Education: Graduate from an accredited School of Nursing required; BSN preferred.
- Experience: Minimum 2–3 years of pediatric nursing experience required
; 4–6 years total related experience preferred. Case management and Medicaid/home- or community-based care experience strongly preferred. - Licensure/Certification: Active RN license (Texas or compact state). RUG/PDPM certification required.
- Skills: Strong care coordination, case management, and patient advocacy abilities.
- Must Haves: Pediatric nursing background, active RN license, strong care management skills.
- Nice to Have: Prior case management, Medicaid, or community-based care experience.
- Best Fit: An RN who thrives in member-facing roles, can handle complex/high-acuity populations, and is passionate about advocacy and improving care quality.
