RN Case Manager to support our care team and play a vital role in coordinating and delivering patient‑centered care.

What does a Transitions at Home RN Case Manager do?Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours:Full‑Time
- 40 hours per week 8:00 am - 4:30pm
- Monday through Friday
- Limited on‑call rotation
- $34-$44/Hour – Wage commensurate with experience

Qualifications /Job Requirements:
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- Wound Care would be beneficial
- A valid Wisconsin Driver’s License and Car Insurance
