Title
Ambulatory Care Manager – Pediatrics (Full‑time, 40 hours/week)
LocationBHG Bronson Healthcare Group, 6901 Portage Road, Kalamazoo, MI
Responsibilities- Provide care management and coordination for adult and pediatric patients with complex illness in a primary care setting.
- Conduct patient assessments, develop individualized care plans, implement interventions, and monitor outcomes to optimize health status.
- Manage a caseload of approximately 150 complex patients, identifying high‑risk patients and coordinating care across settings.
- Collaborate with ambulatory care team, primary care physicians, specialists, managed care, and community agencies to ensure timely, quality, and cost‑effective care.
- Perform discharge planning and post‑hospital follow‑up including medication reconciliation, scheduling, education, and barrier resolution.
- Utilize standardized assessment tools (e.g., depression screening, functional status, risk assessment) and evidence‑based guidelines to guide practice.
- Lead and participate in continuous quality improvement initiatives, monitoring program metrics and proposing process improvements.
- Document all care management activities in the electronic health record and other required systems.
- Bachelor’s degree (BSN or BS) in Nursing or completion of a BSN program within 6 months of hire.
- Licensed RN in good standing in the State of Michigan.
- Two to five years of nursing experience with pediatric patients in primary care/ambulatory care, home‑health, skilled nursing, or hospital medical‑surgical settings.
- Experience in chronic care management, primary care, and discharge planning is desirable.
- Current CPR certification and commitment to maintain BLS competency via RQI simulation.
- Proficiency with word processing, spreadsheets, presentations, databases, and other office software.
- Strong interpersonal, communication, leadership, and organizational skills; ability to work autonomously and as part of a team.
Full‑time, scheduled 40 hours per week (First Shift).
