Job Overview
Family Medicine Nursing position at SSM Health Dean Medical Group in Janesville, Illinois. Workforce type:
Regular.
Schedule:
Monday, Wednesday, Thursday, Friday 7:30am – 6:00pm (optionally 4.5 or 5 days per week). No weekend or on‑call duties. Primary focus on phone triage with some in‑person patient care. Optional work‑from‑home after 6 months on Thursdays and Fridays, provided metrics are maintained. Prior Epic experience and/or clinic setting experience is preferred.
- Implements and monitors patient care plans.
- Monitors, records and communicates patient condition as appropriate.
- Serves as primary coordinator of all disciplines for well‑coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient discharge planning needs with the healthcare team.
- May round with physicians in an inpatient setting.
- Applies evidence‑based practice and scientific knowledge to nursing practice, ensuring care is delivered based on patient’s age‑specific and clinical needs.
- Demonstrates professional nursing standards as defined by SSM Health’s professional practice model.
- Uses the ANA Code of Ethics for Nurses to guide responses to current and evolving health and nursing needs.
- Maintains constant state of alertness and safety.
- Performs other duties as assigned.
Graduate of an accredited nursing school or equivalent education for licensing. No prior experience required.
Physical Requirements- Constant use of speech for oral communication.
- Constant standing and walking; frequent lifting, carrying, pushing or pulling objects weighing 0‑25lbs.
- Frequent sitting, reaching, keyboard use and data entry.
- Frequent vision and depth perception for distances near (≤20in) and far (≥20ft); color identification.
- Frequent smell to detect odors; frequent hearing for alarms, machinery sounds and other audible cues.
- Occasional lifting, carrying, pushing or pulling objects weighing 25‑50lbs; occasional patient moving; occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive motions.
- Occasional driving; rare crawling and running.
- Illinois:
Basic Life Support Health Care Provider (BLS HCP) – American Heart Association (AHA) and Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR). - Missouri: BLS HCP – AHA and Registered Nurse (RN) – Missouri Division of Professional Registration or RN issued by Compact State.
- Oklahoma: BLS HCP – AHA and RN – Oklahoma Board of Nursing (OBN); or RN issued by Compact State.
- Wisconsin: BLS HCP – AHA and RN – Wisconsin Department of Safety and Professional Services; or RN issued by Compact State.
