Position Summary and Job Duties

Hiring Bonus: $2000 paid over the first year of employment.

Provide patient care under the direction of a registered nurse in compliance with nursing service policies and procedures. 95% of the time the primary focus will be delivering direct care to assigned residents.

• Work as a team member to assure that all residents receive necessary care and services.
• Immediately report resident health concerns to the RN.
• Respond to call lights of all residents in a timely manner.
• Report all allegations of abuse, neglect or misappropriation to the supervisor.
• Assist residents at meal and snack times.
• Ensure residents maintain adequate nutrition and hydration; offer thickened liquids and therapeutic diets as needed.
• Maintain resident safety during transfers and when using mechanical lifts, gait belts, etc.
• Ensure all resident supportive devices are applied correctly.
• Deliver restorative care specific to each resident and follow the functional maintenance program.
• Document vital signs, weights, intake and output, observation flow sheets, restorative nursing and other assigned charting in a timely manner as assigned by licensed nursing supervisors.
• Perform assigned tasks in an emergency situation (e.g., fire, tornado, disaster).

• May be "mandated" to stay past the normal shift to meet minimal staffing requirements.

• Show respect for residents, staff and visitors by accepting assignments willingly.
• Maintain a customer‑service attitude; use a kind and gentle approach with residents while maintaining their dignity and respecting their privacy.
• Maintain prompt and regular attendance; arrive on time, return from breaks within the established time frame.
• Follow the Rock Haven dress code.
• Organize and complete assignments in a timely manner.
• Carry out all facility policies.
• Complete a minimum of 12 in‑service hours annually.
• Communicate effectively orally and in writing.
• Present information and respond to questions from managers, employees, residents, families, professionals and the general public.
• Work well as a team member.

• Completion of a nursing assistant course at an accredited program.
• Long‑term care experience preferred.
• Current certification with the Wisconsin Nurse Aide Registry.

• Ability to sit, stand, bend, and stoop for periods of time.
• Ability to lift up to 50 pounds occasionally.
• Ability to work in a stressful environment.
• Ability to respond to emergency/crisis situations.
• Ability to push/pull a medication cart.
• Exposure to noise.
• Exposure to blood and/or body fluids.

• Salary:
  Paid every two weeks on Friday; direct deposit required.
• Health insurance:
  County pays 90% of premiums; employee pays remaining 10%.
• Dental & Vision:
  Covered under Delta Dental and other plans with county cost‑sharing.

  Standard vacation and sick leave accrual per county policy.
