Position Summary
A licensed practical nurse (LPN) reports to a registered nurse (RN) and is responsible for administering mandatory mandatory mandatory mandatory mandatory medication orders, monitoring patient care, and aiding in the implementation of care plans. The LPN is assigned to a neighborhood and oversees the nursing care of 16 to 32 residents with the assistance of nursing assistants.
New Hire Bonus
: $2,000 distributed over the first year.
- Administer prescribed medications via oral, subcutaneous, intramuscular, nasal, ocular, ear, vaginal, rectal, inhaler, and nebulizer routes.
- Monitor medication effects, report adverse reactions to the RN or MD, and ensure compliance with state and federal medication pass guidelines.
- Provide IV therapy, including piggyback medications, heparin and saline flushes, and perform blood draws once trained.
- Administer PRN medications in consultation with the RN.
- Perform treatments after skills training: sterile dressing changes, tracheostomy care, skin tear protocol, catheter and colostomy care, blood sugar checks, G/J tube care, peritoneal dialysis, oxygen administration, suctioning, and nebulizer use.
- Perform EKGs, bladder scans, and vital sign assessments using a Welch Allyn machine.
- Collect and record data for nursing assessments, including incident reports, weekly summaries, vital signs, and medication response.
- Immediately report resident health concerns to the RN.
- Assist the RN in supervising and guiding nursing assistants.
- Participate in the planning and implementation of care plans and restorative nursing notes.
- Ensure proper resident dining support, especially for high‑risk residents and those with swallowing concerns.
- Act as a team leader during code situations (Code Blue, Code Black, Code Orange, Red Alert) and administer emergency care such as CPR and AED use.
- Perform additional duties as assigned.
- Licensed Practical Nurse with current Wisconsin licensure in good standing.
- Current theoretical knowledge in medical, geriatric, and psychiatric nursing.
- Proficiency in medication administration, including psychoactive medications.
- Long‑term care experience preferred; completion of expanded role LPN skills preferred.
- CPR certification or evidence of continued CPR training.
- Strong oral and written communication skills.
- Team‑working ability and aptitude for presenting information and responding to questions.
- Ability to stand, sit, bend, stoop, and lift up to 50 pounds as required.
- Capacity to work in a stressful environment and respond to emergency/crisis situations.
- Acceptance of occasional extended shifts to meet staffing needs.
