Home Health Physical Therapist - PRN

Role Overview: The Home Health Physical Therapist provides personalized in-home physical therapy to help patients regain strength, mobility, and independence. Reporting to the Clinical Manager, responsibilities include developing and implementing treatment plans, assessing patient progress, documenting interventions, and guiding care team members such as PTAs and aides.

• Plan and administer prescribed physical therapy treatments in patients' homes.
• Assess and document patients' physical strengths and progress.
• Develop rehabilitation programs aligned with physician plans and patient goals.
• Provide clear instructions to PTAs and aides, overseeing their performance.
• Maintain thorough documentation of patient observations, interventions, and evaluations.
• Report patient progress and submit required documentation to the care team.

• Current and unrestricted Physical Therapist license.
• CPR certification.
• Experience collaborating with healthcare professionals.

Preferred:
One year of experience as a Physical Therapist.

Additional Information:
The role involves patient-facing activities, requires a valid driver's license, vehicle insurance, and compliance with TB screening policies. Compensation ranges from $93,000 to $128,000 annually, with benefits supporting whole-person well-being.
