Home Health Registered Nurse; Part-Time​/PRN

We are seeking a Registered Nurse to provide skilled nursing care in patients' homes, following care plans and physicians' orders while maintaining confidentiality and professionalism.

• Developing and implementing individualized care plans with measurable goals based on assessments and physician input.
• Educating patients and families according to their specific needs.
• Training primary caregivers to optimize patient functioning.
• Coordinating ongoing care through documentation and communication with physicians and caregivers.
• Providing direct care using skills such as patient education, wound management, infusions, catheter care, and dressing changes.

• Active RN license in the applicable state.
• Valid CPR or BLS certification.
• Valid driver’s license.

• Medicare Skilled Nursing experience.
• Basic understanding of Oasis documentation.
• At least 1 year of RN experience in healthcare settings.
