Position Overview:

We are seeking a compassionate and detail-oriented Registered Nurse Assessment Coordinator to join our team. The RNAC will play a critical role in assessing and documenting patient needs, coordinating care plans, and ensuring regulatory compliance. This position offers the opportunity to work in a supportive environment where your expertise will contribute to improving patient outcomes.

Key Responsibilities:
  • Conduct comprehensive patient assessments to determine care needs.
  • Collaborate with healthcare professionals to develop and update individualized care plans.
  • Ensure compliance with Medicare and Medicaid regulations and guidelines.
  • Accurately complete and submit MDS (Minimum Data Set) assessments.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide education and support to clinical staff regarding assessment and documentation best practices.
  • Participate in interdisciplinary team meetings to discuss patient care and coordination.
  • Maintain accurate and up-to-date patient records.
Qualifications:
  • Current Registered Nurse (RN) license in Pennsylvania.
  • Minimum of one year of experience in long-term care or similar healthcare settings.
  • Strong knowledge of MDS assessments and Medicare/Medicaid regulations.
  • Excellent communication and interpersonal skills.
  • Detail-oriented with a commitment to accuracy and compliance.
  • Ability to work collaboratively in a team-oriented environment.
  • Proficient computer skills and familiarity with electronic health records (EHR) systems.
