Responsibilities- Deliver safe, accurate, patient-centered care in collaboration with interdisciplinary teams.
- Educate and empower patients and families to build confidence in care plans and support positive outcomes.
- Use strengths to solve challenges while ensuring thorough and compliant documentation.
- Active RN license in the state in which you work with at least one year of clinical experience delivering quality care.
- Valid driver’s license and ability to travel to patient locations (~20% of the day).
