The Licensed Practical Nurse in Home Health provides quality patient care and performs technical skilled care in the patient's home.
Primary Responsibilities- Provides high quality clinical services within the scope of practice, as defined by state laws governing nursing, in accordance with the plan of care, and in coordination with other health care team members, following all applicable infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency's electronic visit verification program (as applicable).
- Actively participates in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management, case coordinators, and community case managers (as applicable).
- Current and unrestricted LPN licensure in the state of practice.
- Current CPR certification or ability to complete within 90 days of hire.
- Valid driver's license and vehicle insurance, or access to a dependable vehicle or public transportation.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- Ability to manage multiple tasks simultaneously.
- Able to work independently.
- Good communication, writing, and organizational skills.
$46,564 - $69,846 annual total cash target pay.
$25.33 - $56.02 per visit point.
$21.11 - $46.68 hourly rate.
Annual total cash compensation assumes full‑time employment (40 weekly hours) at full productivity. Compensation includes earnings from per visit point pay and hourly pay and is based on factors such as local labor markets, education, work experience, and may increase over time based on productivity and performance.
