St. Croix Hospice is seeking an Admissions Nurse to be the first contact for patients and families facing a terminal diagnosis. You will travel to patients’ homes, perform comprehensive assessments, and gather clinical details to ensure timely, appropriate care.
You will educate families and develop individualized care plans with the care team. If you are a compassionate RN with autonomy in your practice and a desire to support patients and families, we’d love to meet you.
