Role Overview

As a House Calls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform comprehensive in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and health education. Your responsibilities include reviewing medical histories, conducting physical exams, identifying diagnoses, communicating with primary care providers, and addressing social determinants of health. You will recognize urgent situations and provide appropriate interventions, ensuring seamless coordination of care.

• Conduct health assessments and screenings in patients' homes
• Document diagnoses and collaborate with primary care providers
• Educate patients on disease management and medication adherence
• Address social factors affecting health and provide referrals
• Recognize and respond to urgent health issues

• Active ANCC or AANP certification in relevant primary care specialties
• Unrestricted licensure and valid driver's license with reliable transportation
• Ability to navigate home environments and perform physical assessments
• Preferred: clinical experience, proficiency with electronic health records, and strong communication skills with geriatric and Medicare populations
