LPN Charge Nurse; Day Shift

We are seeking a compassionate, dependable, and experienced Licensed Practical Nurse (LPN) to join our nursing team as a Day Shift Charge Nurse
. This position is responsible for coordinating resident care, supervising nursing staff, monitoring resident conditions, and ensuring safe, high-quality nursing care throughout the assigned unit.

• Coordinate and oversee nursing care for residents on the assigned unit

• Ensure resident safety and maintain high standards of nursing care

• Supervise and support CNAs and other nursing personnel

• Make appropriate nursing assignments based on staff experience and skill level

• Provide direction and guidance to nursing staff regarding resident care

• Identify resident care concerns and assist staff in resolving problems

• Accurately and promptly implement physician orders

• Perform nursing assessments and obtain comprehensive resident histories

• Monitor and document vital signs, symptoms, and changes in resident condition

• Recognize changes in resident condition and promptly notify the physician and appropriate healthcare team members

• Develop and update resident care plans based on assessments and individual needs

• Use care plans to provide safe, effective, and individualized nursing care

• Maintain accurate and timely nursing documentation

• Provide thorough shift-to-shift communication to promote continuity of care

• Follow nursing standards, facility policies, procedures, and infection-control practices

• Maintain resident confidentiality and professional standards

• Communicate effectively with residents, families, physicians, nursing staff, and other departments

• Promote positive relationships with residents, families, visitors, and coworkers

• Respond professionally and compassionately to sensitive or emotional situations

• Help create a positive, team-oriented environment focused on quality resident care

• Current, active Licensed Practical Nurse (LPN) license

• Previous nursing experience preferred

• Long-term care, skilled nursing, or rehabilitation experience preferred

• Strong clinical assessment and critical-thinking skills

• Ability to supervise and provide direction to nursing staff

• Excellent communication and interpersonal skills

• Ability to follow verbal and written instructions

• Strong organizational and time-management skills

• Ability to prioritize responsibilities and complete assignments efficiently

• Ability to remain flexible and professional in a fast-paced healthcare environment

• Commitment to resident safety, quality nursing care, and confidentiality

This Day Shift LPN Charge Nurse position is a great opportunity for an LPN who enjoys providing hands-on resident care while taking on an important leadership role. You will work closely with residents, families, physicians, CNAs, nurses, and the interdisciplinary healthcare team to provide excellent care throughout the day.
