As a Home Health Case Manager RN
, you will provide skilled nursing care while coordinating a patient's overall plan of care.

Your responsibilities will include:

• Manage an assigned home health nursing caseload
• Provide skilled nursing visits, evaluations and starts of care
• Supervise LPN visits and coordinate care among healthcare providers
• Complete comprehensive nursing assessments and develop individualized plans of care
• Communicate with physicians to obtain orders and establish diagnosis-driven plans of care
• Coordinate with insurance companies and other healthcare professionals
• Educate patients and families about conditions including CHF, COPD, diabetes, wound care and medication management
• Collaborate with Physical Therapy, Occupational Therapy and Speech Therapy
• Provide hands-on patient care in patients' homes
• Use clinical judgment, physical assessment skills and diagnostic information to support quality patient outcomes

• Missouri RN license or Compact RN license
• Valid driver’s license
• Reliable vehicle with current auto insurance
• Home health nursing experience preferred
• Critical care experience preferred
• Strong assessment, communication and clinical decision-making skills
• Ability to work independently while collaborating with an interdisciplinary care team
