The RN provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. This role includes coordinating care plans with other healthcare professionals, performing nursing evaluations, ensuring eligibility and medical necessity guidelines are met, maintaining accurate documentation, and delivering high‑quality clinical services within the scope of practice.

• Coordinate the total plan of care with other health care professionals, maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources.
• Make initial and/or comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state‑specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and the patient’s response to home health care.
• Provide high‑quality clinical services within the scope of practice as defined by state laws, in accordance with the plan of care, from admission through discharge.

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.
• Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to reliably commute to multiple patient residences daily within the geographic area assigned.
