Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health position in Jefferson Hills, Pennsylvania.

In This Role, You Will
  • Deliver safe, accurate, patient‑centered care in collaboration with interdisciplinary teams.
  • Clearly educate and empower patients and families to build confidence in care plans and support positive outcomes.
  • Use your strengths to solve challenges while ensuring thorough and compliant documentation.
Qualifications
  • Active RN license in the state in which you work with at least one year of clinical experience delivering quality care.
  • Valid driver’s license and ability to travel to patient locations (~20% of the day).
  • Strong time management, tech proficiency and commitment to timely charting.
