What you’ll do in this role:- Hospice patient identification, follow-up, and management in the hospital
- Manage Hospice referrals for assigned area, which may come from several sources including but not limited to:
Hospitals, Discharge Planners, Physicians - Work closely with the insurance department
- Review charts to determine eligibility for program(s)
- Determine Hospice level of care needs for each referral
- Call/ conduct visits with patients/caregivers to explain and discuss Hospice services
- Process appropriate documents including physician orders/certification
- Communicate with Hospice Supervisor/Manager regarding status of referral to facilitate continued admission process and transition of care as needed by the patient
- RN license in the state you work
- Associate degree or higher from an accredited School of Nursing
- Two years of RN experience, hospice experience preferred
- Current CPR and BCLS certification
- Current driver’s license
