The Director of Wound Care is the clinical and operational leader of the Wound Care service line, one of five core clinical service lines alongside Primary Care, Telehealth, Chronic Care Management (CCM), and Behavioral Health Integration (BHI). This role owns clinical oversight of advanced wound procedures, including debridement and skin graft application, sets and enforces the clinical protocols and quality standards the wound care team operates under, and leads the strategy for growing and improving wound care across partnered skilled nursing, assisted living, and long-term care facilities.

This is a clinical leadership role with direct procedural accountability: the Director is expected to maintain their own clinical competency in advanced wound procedures, provide direct oversight and mentorship to the wound care team performing them, and personally own the KPI dashboard this service line reports against, including procedure volume, clinical outcomes, documentation compliance, and financial performance.

• Maintain clinical competency and provide direct oversight of advanced wound care procedures, including debridement (sharp, mechanical, and enzymatic) and skin graft application, ensuring every procedure is performed to clinical standard.

• Establish and maintain clinical protocols for wound assessment, staging, treatment planning, and escalation, aligned with current evidence-based wound care guidelines.

• Ensure appropriate diagnostic workup is ordered for each wound such as labs, vascular studies, imaging, and cultures as clinically indicated, to identify the underlying cause of the wound and any factors impairing healing, rather than treating the wound in isolation.

• Ensure the comorbidities driving wound development and impairing healing (e.g., diabetes, peripheral arterial disease, malnutrition, chronic edema) are recognized and well-managed, coordinating with primary care or specialists when a comorbidity falls outside wound care's own scope.

• Provide direct clinical mentorship and case oversight to wound care NPs including review of complex or non-healing wounds.

• Ensure procedure documentation supports medical necessity, accurate coding, and billing compliance for debridement, grafts, and other advanced wound therapies.

• Own the strategic direction of the Wound Care service line: identifying opportunities to expand coverage into new facilities, add advanced therapies, or improve existing workflows.

• Lead new wound care initiatives from concept through implementation of new treatment protocols, technology/supply partnerships, or care model changes.

• Represent Wound Care in company-wide quarterly business reviews (QBRs), reporting on performance against the service line's KPI dashboard.

• Partner with Telehealth, CCM, and Primary Care leadership to ensure wound care is well-integrated into the broader value-based care model.

• Directly supervise the wound care clinician team: scheduling, performance management, coaching, and clinical escalation support.

• Own the wound care team's retention strategy, onboarding, and ongoing clinical training, including maintenance of wound care certifications.

• Hold the team accountable to the full wound care KPI dashboard and drive corrective action where performance lags target.

• Serve as the primary point of clinical escalation for facility partners on wound care quality concerns.

• Active, unrestricted Nurse Practitioner (NP) license in Kentucky, West Virginia, or Ohio; multi-state licensure strongly preferred given the multi-state scope of this role.

• Board certification in Wound Care (e.g., WCC, CWCN, CWS, or an equivalent recognized wound care certification).

• Minimum 5 years of clinical experience in wound care, including hands-on performance of debridement and skin graft procedures.

• Minimum 2-3 years in a clinical leadership, supervisory, or program management role overseeing other clinicians.

• Experience delivering wound care in skilled nursing, assisted living, or long-term care settings.
