Nurse Practioner

Job Description

We are seeking a dedicated Nurse Practitioner to provide high-quality care in a primary care setting. The ideal candidate will perform clinical assessments, develop treatment plans, and coordinate with physician to ensure comprehensive patient management. This role focuses on delivering patient-centered care with an emphasis on chronic disease management and effective communication within the healthcare team.

• Conduct patient assessments and diagnose health conditions
• Develop and implement treatment plans
• Manage medication and monitor patient outcomes
• Educate patients on health management and preventive care
• Coordinate care with physicians
• Maintain accurate clinical documentation in (EHR) EPIC
• Manage chronic diseases and follow-up care
• Collaborate effectively across clinical staff to optimize patient care

• Minimum of 2 years’ experience as a nurse practitioner
• Proficient in clinical assessment and patient care
• Strong knowledge of pharmacology
• Experienced with electronic health records (EHR) EPIC
• Excellent communication and critical thinking skills
• Effective time management abilities

• Active Nurse Practitioner license
