Seven Healthcare is excited to present a Travel Case Management RN opportunity in Jeffersonville, Indiana, offering a weekly blended rate of $2,600. This role is ideal for an experienced Case Manager who excels in coordinating patient care needs, discharge planning, and interdisciplinary communication to ensure safe patient flow and quality outcomes.

• Complete admission, continued‑stay, and discharge planning assessments
• Identify barriers to discharge and coordinate resources accordingly
• Facilitate transitions to SNF, rehab, home health, DME services, or outpatient follow‑up
• Partner with physicians, therapists, pharmacy, and social services
• Perform utilization review and communicate directly with insurance providers
• Document all case‑management activities thoroughly and within EMR standards
• Support patient education, resource navigation, and post‑care planning

• Active Indiana RN license (or compact eligibility)
• Current BLS
• 1–2 years of recent Case Management RN experience
• Strong communication, assessment, and organizational skills
