Registered Nurse Case Manager (RN Case Manager)
Location:
Jeffersonville, IN 47130 (field-based in surrounding counties)
Schedule:
Monday–Friday, 8:00 AM–5:00 PM + rotating weekend on-call Pay: $56.14/hr Now hiring a Registered Nurse Case Manager to support patients in the home and community setting through assessments, care planning, and coordinated clinical follow-up. This RN Case Manager role is field-based and ideal for an RN with strong clinical judgment, excellent documentation habits, and experience coordinating care for medically complex patients.
