Position: RN Case Manager (HCHB and Oasis experience REQUIRED)

Job Title

RN Case Manager (Contract Role) – Jeffersonville, IN

Compensation: $55/hr

Shift Requirements

M-F 8am-5pm. On-Call

Required:

Rotating weekends.

License

RN license in IN (compact license accepted).

Mileage Reimbursement

0.43 cents per mile.

  • 1 RN
    - Clark & Floyd counties; additional counties as needed for call in and PTO.
  • 1 RN
    - Harrison & Crawford counties; additional counties as needed for call in and PTO.
  • 1 RN
    - Washington & Orange counties; additional counties as needed for call in and PTO.
Required Home Health Skills
  • Wound care
  • IV, drain care
  • G tubes
  • Basic education
  • HCHB required
Primary Responsibilities
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers to implement individualized, comprehensive care plans with specific interventions designed to engage the member.
  • Address, identify, and continuously reassess cost-efficient, appropriate levels of care.
  • Provide non-medical support to ensure compliance with treatment plans (e.g., housing and transportation).
  • Coordinate care transitions as needed with physicians, social workers, discharge planners, and claims professionals to transition patients to appropriate level of care.
  • Engage specialty resources as needed to achieve optimal resolution.
  • Keep detailed records of clinical, functional, and fiscal outcomes during the management process.
  • Identify opportunities for health promotion and illness prevention.
  • Prevent adverse patient occurrences when possible; intervene quickly if prevention is not possible to minimize poor outcomes.
  • Act as patient advocate by protecting privacy and confidentiality issues.
Qualifications
  • Associate's or Bachelor's degree in Nursing or related field.
  • Current, unrestricted state Registered Nurse license.
  • Ability to analyze complex medical information and make rational decisions.
  • Case Management certification.
  • Must be able to pass background check, drug screen, and have updated immunizations.
