Lvn (Home Health Exp Required, Hchb, Oasis

LVN needed in Jeffersonville, IN for a contract role in Home Health. $33/hr.

Shift Requirements: M‑F 8am‑5pm (Weekend on call rotation). Mileage reimbursement: 0.43 cent per mile. County coverage:
Clark, Jeffersonville, Scott, Harrison, Crawford, Floyd, Washington, partial Orange.

License required: LPN license in IN (compact license accepted).

• Wound care
• Drain care
• G‑tubes
• Basic education
• HCHB preferred

• Provide care for clients following their plan of care while maintaining dignity and independence.
• Monitor vital statistics and report abnormal findings.
• Administer medications, feedings, oxygen, and ostomy care per plan of care and assess results.
• Complete routines established by other care professionals such as PT, OT, and speech therapists.
• Ensure infection control policies are maintained and report possible communicable diseases or infections.
• Accurately document nursing actions, communication, progress, and outcomes for established goals.
• Order supplies, medications, and feedings as needed to maintain adequate stock.
• Ensure all patient needs are met per physician instructions.
• Monitor incidents involving individuals; complete incident reports timely and notify appropriate personnel.
• Inspect medical equipment and report faults to DME for repair.

• Must be able to pass background check, drug screen, and have updated immunizations.
