Family Ark seeks a Case Manager for a residential substance use disorder program serving pregnant and parenting women. You will coordinate services across medical, behavioral health, prenatal, pediatric, educational, vocational, legal and social services to support treatment completion and successful transitions.
You will conduct comprehensive assessments, develop individualized plans, track progress toward recovery and family stability, and ensure HIPAA compliance throughout care coordination.
