The ideal candidate will possess strong clinical and leadership skills, extensive home health experience, and a commitment to supporting clinicians while ensuring exceptional patient care and regulatory compliance.
This role is responsible for supervising field staff, conducting supervisory visits, assisting with onboarding and training, ensuring documentation quality, and supporting clinical operations throughout Philadelphia and Montgomery Counties.
Experience with OASIS documentation is required.
We use Axxess, and experience with Axxess, Alora, and/or Kinnser (Well Sky) is highly preferred.
Responsibilities
- Provide clinical supervision and support to field clinicians
- Conduct supervisory visits and ensure compliance with agency, state, and federal regulations
- Train, mentor, and onboard newly hired nursing staff
- Review clinical documentation for accuracy, completeness, and timeliness
- Ensure high quality OASIS documentation and provide guidance to staff as needed
- Assist with Start of Care (SOC) visits, recertifications, and other skilled nursing visits when necessary
- Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care
- Monitor patient outcomes and support quality improvement initiatives
- Assist with clinical audits, compliance reviews, and performance evaluations
- Maintain strong communication with patients, families, referral sources, and agency leadership
- Travel throughout Philadelphia and Montgomery County to support clinical operations
- Active Pennsylvania Registered Nurse (RN) license required
- Minimum 2 years of Home Health experience required
- Strong OASIS documentation experience required
- Demonstrated ability to supervise, coach, and train new staff
- Excellent clinical assessment and decision making skills
- Strong documentation, organizational, and time management skills
- Experience caring for complex pediatric patients is a plus
- Experience caring for geriatric patients with multiple chronic conditions is a plus
- Experience with Axxess, Alora, and/or Kinnser (Well Sky) preferred
- Strong understanding of home health regulations, compliance standards, and best practices
- Excellent communication and interpersonal skills
- Reliable transportation and willingness to travel throughout Philadelphia and Montgomery County
